GENERAL RELEASE OF LIABILITY I for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release and hold harmless 905 Game Zone, its officers, officials, agents, and employees, from any and all liability for injury, disability, or loss/damage to personal property. I freely assume all such risks, both known and unknown, and take full responsibility for myself and all participants listed above.I have read and understood the rules and agree to fully comply during participation.

If the party is held on property owned by an individual or organization other than the participant(s), it is understood that the event is in no way sponsored by the property owner. The property owner and their representatives assume no responsibility for the conduct or safety of the event/activity. Participants shall hold harmless the property owner from any cause of action, claim, or petition arising from the use of the property in relation to 905 Game Zone.

RATING RELEASE We take all reasonable precautions to ensure children are not exposed to inappropriate material. However, by signing below, you agree to release 905 Game Zone, including its directors, employees, staff, and sponsors, from liability arising from any such exposure.I certify that I am the parent/legal guardian of the above-named and that I accept full liability and responsibility for their actions, including anything resulting from the use of equipment and products owned, operated, sold, or provided by 905 Game Zone.

PHOTO, VIDEO, & SOUND RECORDING RELEASE During events or parties, photographs, video, and/or sound recordings may be captured. These may be used to promote 905 Game Zone activities, services, or programs, and may also be taken and used by others present at the event. By signing this release, you grant permission to 905 Game Zone, and any persons present, to use, reproduce, and/or distribute such images, videos, or sound recordings on websites, social media, or print publications. These may be used for an indefinite period.905 Game Zone does not sell or distribute captured media to third parties; however, once posted online, the content may be accessible to anyone.I acknowledge, understand, and agree to assume all risks associated with this, and sign this release of liability voluntarily and without inducement.

MINOR PARTICIPANT CONSENT All players under the age of 18 must have a parent or guardian sign below. I certify that I take legal responsibility for the above-listed participant(s) and/or all party participants as indicated below. I agree to indemnify 905 Game Zone and all associated individuals from all liabilities arising from their participation.
